Identify key milestones that you expect to achieve, monitor and track to measure success.
This is an initial list of key milestones that will need to happen to get project up and running:
• Collect and analyze current baseline data to determine goals
• Discuss with key leadership to ensure buy in
• Meet with staff to determine their roles and impact
• Set up “community referral” training schedule
• Set specific tasks and timeline for project – including setting “task alerts” for staff within Fedcap Cares
• Monitor data reports at least weekly and meet with staff to share progress and their next steps
Will you change course during start-up or implementation based on what you learn? If so, in what ways?
I suspect there will be many course corrections during implementation and start up. I will constantly monitor data-including number of referrals we make and look at the outcomes. I plan to meet with staff each week to share this information and work with them to identify the participants that still need more contact and follow up on the referral. If I see that the process is not working—that we are not seeing positive outcomes from the work we are doing—I will alter what we are doing.

I will be responsible for tracking success and challenges. I anticipate that I will monitor at least on a twice-weekly basis but may change that as I get further into the project.
Are there any other strategies you will devise to ensure project success?
I will continue to assess the project using critical thinking skills and constantly monitoring the data that I receive. I will check in with staff frequently to ensure they understand their roles and remind them of the impact they are having on families lives here in Maine. Creating a can-do attitude and culture here in the office is important for this project.
